The Surgical Approach to Adult Total Anomalous Pulmonary Venous Connection: Case Report

Total anomalous pulmonary venous connection is a very rare congenital cardiac anomaly of all cardiac anomalies. Syptoms are often seen after the early neonatal period. The presence of large atrial septal defect (ASD) is necessary in all adult persons without underwent any surgical procedure. The patient who underwent surgical operation after the diagnosis of total anomalous pulmonary venous connection, admitted to our department for routine controls without any symptoms. Atriotomy was done to the posterior wall of left atrium and venotomy to the common trunk of pulmonary veins and then anastomose to each other by using 5/0 polypropylene suture. ASD was closed with pericardial patch. Large vertical vein was ligated by dacron tapes to prevent the connection between pulmonary veins and right atrium. The patient was discharged without any complication folowing the period of intensive care unit and postoperative unit. The results of operations are usually satisfactory but in some literatures hospital mortality was 2-20 %. Acidosis is the major risk factor for early deaths. Anastomotic stricture, the pulmonary vein stenosis due to diffuse fibrosis and thickening of the vein wall are the most serious complications during the late postoperative period.

We believe that because of rarity, and postoperative early and late complications total anomalous pulmonary venous connection should not be underestimated.
