Index of myocardial performance in patients with type II diabetes mellitus without hypertension and its relationship with serum amylin levels and echocardiographic parameters

Aim:

Type 2 diabetes mellitus (T2DM) affects the systolic and diastolic function of the lenf ventricle. The serum levels of amylin (Islet Amyloid Polypeptide (IAPP)) are significantly higher in T2DM patients. The aim of the present study was to evaluate the IAPP that contributed the diabetic cardiyomyopathy and its relationship with myocardial performance index (MPI), cardiac and vascular parameters in asymptomatic normotensive patients with T2DM.

Material and Methods:

Study population consisted of 51 asymptomatic normotensive patients with T2DM (19 women, 32 men; mean age 48 ±12). M-Mode, two-dimensional Doppler echocardiography and carotid Doppler ultrasound was performed in all subjects to calculate MPI and carotis intima-media thickness. Venous blood samples were collected for analysis as insulin, c-peptide, HbA1C, free T4, post prandial blood glucose, 24 hours urine albumin and serum amylin.

Results:

There was a statistically significant positive correlation between serum IAPP level and MPI in T2DM patients (r = 0.443, p <0.05). There was a positive correlation between serum levels of IAPP and carotis intima media thickness (r = 0.295,p <0.05). Statistically significant negative correlation was found between serum IAPP levels and age (r=-0.454,p<0.05).

Conclusion:

Our findings suggest that elevated serum IAPP levels is associated with increased MPI, elevated serum IAPP levels may be predictor for an early sign of diabetic related cardiomyopathy in normotensive T2DM patients with preserved LV function.
